Parental / Guardian Consent (Version 1)

I, [guardian_name], parent or legal guardian of [athlete_name] (date of birth: [dob]), hereby give explicit consent to the Maharashtra Aquatic Association (MAA) to collect, store, and process my child's personal data through the SwimSetu platform for the purpose of swimming registration, district affiliation, and related sporting administration.

I understand that this includes: name, date of birth, gender, district, parent names, contact information, address, and upload of identity documents (e.g. birth certificate). Data will be stored on servers located in India (Mumbai and Bangalore).

I understand that MAA district administrators will review this registration and that I may withdraw consent or exercise data rights by contacting the Grievance Officer at [GRIEVANCE_EMAIL], subject to retention requirements for approved sporting records.
